I have a 3-year old Ragdoll, Fog, a neutered male, who had been the runt of the litter and has never grown to his expected size. One year ago, he lost a significant amount of weight-almost 2 pounds, bringing him down to about 8.5 pounds. He had been just over 10 pounds at his first well visit just after he had turned 1 year old. He was given every conceivable test-urine, blood, x-ray, ultrasound, and even a barium test to see if the problem lay in his esophagus. All tests have been normal. Prior to the ultrasound and barium test, we gradually changed his diet to Science Diet z/d under the assumption that he has a food allergy. (He had been eating Wellness hard food.) After 4-5 months on the z/d, he remained thin and continued to regurgitate. Vet suggests exploratory surgery next. Would it be okay to administer prednisone assuming it's IBS before preceding with surgery?

Hi XXXXXXXX,
Welcome to Just Answer. At this point, if you are going to go through with the exploratory on Fog, most definitely "No" on the administering of prednisone prior to it. The reason for this, your doing the surgery to get tissue to get an answer. The use of the prednisone may alter the tissues and make obtaining a diagnosis more challenging or impossible. I would say, after the surgery, a lot of these patients are started on it in lieu of the results coming back - but not before.

I was hoping to be able to forgo surgery-should the prednisone yield a good result then couldn't we conclude that it is IBS? Yet I understand that should he be harboring a slow-cell or small- cell lymphoma, it could take a longer time for him to be really sick. Do you know how risky the surgery is? My vet said that either possible cause for the weight loss, cancer or IBS, could show an absorption problem. Can I assume if the prednisone doesn't work, it will take a long time for it to leave his system before proceeding with an exploratory? Should the surgery reveal that Fog has a cancer, then what? Besides these two conditions, can the surgery reveal anything else?

<p>Ok. This is a situation where I'll put this scenario out there for an owner. If it can definitely be proven that your little one has lymphoma, would you do a more complete chemotherapy protocol other than oral steroids. The reason I pose this question is that if an owner says they won't do any other chemotherapy besides the prednisolone, then I will give the green light for a trial of steroids. If response to therapy is good, it is continued until things aren't going well if that occurs. If always doing good, assume it is the IBD situation. If they deteriorate in weeks to months, assume it is the lymphoma. At that point then do they do more tests to prove it. That is the owners choice.</p><p> </p><p>The surgery itself shouldn't be that risky if done by an experienced vet with appropriate equipment and staff. </p><p> </p><p>Dr. Bruce</p>
