Question

My 14 year old Tonkinese cat recently had an Ultra Sound because of a large lump by his kidneys. They extracted fluid from this area also. The teat came back he did not have cancer, but has an inflamed kidney. We will now start him on anti-biotics. If they don't work they cannot excrete fluids anymore a kidney removal would be next but because of his age this is not an option.

Are there any Vitamins I can give him? I still must give him the IV fluids every other day.
Any change in foods?? He is on K/d and chicken baby food and vitamins. Can I give him sweet potatoes, rice macaroni for weight gain? Any suggestions?

Answer

Hello and thanks for researching this very important question!

The foods you have suggested may be cooked and offered in small amounts...too much change too quickly can cause digestive upset, so go slow.

It does not seem clear whether your pet's kidney is failing itself, or if it is reacting to the "mass" found nearby. If both of his kidneys are aging and "naturally" losing their filtering ability, then you will need to manage him day-to-day from now on.

Cats with kidney disease should have blood tests run to find out exactly how much renal function is left, and to determine if other metabolic problems exist.

If too much nitrogen (a metabolic by-product) is building up in his blood, he will feel sickly and have many problems with his digestion and even brain function. There is a new supplement that has shown good early results with cats.

Some cats with kidney disease have trouble with too much phosphorous in their blood and/or not enough potassium. Supplements can be used to address these problems as well, but you should make sure that his body needs them...find out what his blood test results say about these compounds.

Read about basic supplements here.

One of the jobs the kidneys do is to kind of "count" the red blood cells. Once red blood cells fall below a "set" point, the kidneys secrete a hormone that tells the bone marrow to make new blood cells. When the kidneys are sick, they can lose this ability. Anemic cats can be treated with hormone injections as needed. This therapy is tracked with periodic blood cell counts.

If he seems like he's having a hard time with appetite, make sure he is not anemic first, then discuss whether some antacid medicine might be helpful. Nitrogen in the body can damage the digestive linings and make the stomach and intestines very sensitive and even ulcerated.

I have been giving my cat Alternagel 2 times a day. Hi-Vite Drops
Felovite 11. And EEl Serum. But now that we know what his problem is
things good change. He was Anemic last month but his recent blood test show it is normal.

The inflamation of the kidney I would like to do the reigh thing as to not have this happen again.

Please advise.

It really depends on what inflammed the kidney to begin with...if there is a problem in his body that cannot be fixed, then you will always have to provide support as you are doing now.

Periodic blood tests---every one-to-two months if he feels OK at home, or more often if he starts to feel sick again---will help guide your supplements and treatments.

It is a good sign that his anemia has corrected itself, at least for the time-being. This means that his "good" kidney is strong enough to do all the work, or that his inflammed kidney is feeling better.

If he tolerates the fluid treatment at home and does not have increased blood BUN or creatinine levels, then I'd discuss with the vet if more protein in his diet would be better now...if he is needing it, his body will steal it from muscles and cause weight loss. Basically he would start to digest himself.

It makes a big difference to know if your vet has decided that your pet's kidneys are trying to cope with some other problem and are stressed-out temporarily (and need a lot of support for now to help them heal), or if they are getting old and slowly deteriorating (and will not get better, but need to be managed all the time from now on, as you are already doing).

This article tells how to treat and manage cats with sudden ("acute") kidney distress:
Read about Acute Renal Failure here.

And this one talks about the progressive (old-age) form of kidney disease ("chronic"):
Read about Chronic Renal Disease here.

Let me know if these articles help you...it does sound as though your vet is quite familiar with the treatments to use and blood tests to monitor in this kind of case.
