Question

My 10 wk old has been suffering from reflux since Friday. I noticed him whimpering (didn't seem to be in pain though), which I had never seen him do before. That night he had two episodes of choking badly on his spit-up. He showed minor symptoms of reflux at 4 weeks, but this only included burps and gas and minor discomfort here and there. Now he is drooling, coughing, showing definite pain (I can hear the reflux which I never did before last Friday), & difficulty sleeping. My older son had silent reflux as a baby & we now know he has major food allergies. I am off of dairy b/c it seemed to bother our newborn. I am breastfeeding him. He had his vaccines last Monday. I was also on Cefaroxil for a staph infection until last Friday (the same day his symptoms arose). No other changes to my diet or his sleep/eating schedule. If this is reflux, why is it all of a sudden manifesting at such a late age? Any other explanations for what is going on, what could be causing this or how to help him?

Thank you for writing in today. I want to provide you with the best possible information, so I do have a couple of questions. I hope you understand.

When he appears to be in pain, do you notice any arching of his back?What is his height and weight trend on the curve? Normal?
Any symptoms of thrush in the child, such as white plaque in the mouth or a new diaper rash? Also, any syptoms of thrush around your nipples?

He doesn't arch his back, but he does stiffen a bit. He is in 50th for weight & 75th for height. I forgot to ention he was 4 weeks early - 6 lbs 13oz at birth & completely healthy. He has some white plaque on tongue but dr. didn't see any elsewhere & I have no signs of it. No new diaper rashes.

<p>Hello;</p><p> </p><p>Thank you for responding. Also, before I begin, I apologize for the HTML coding appearing in the response. It is a technical glitch that we are currently working on.</p><p> </p><p>First of all, let's address the GERD concerns. Gerd can appear at this age, and one resource mentions a peak around 4 mths. of age. However, I'm more interested in the timing of his symptoms and your recent use of antibiotics. That particular antibiotic is transmitted via breast milk. Now, there has never been any harm associated with use during breastfeeding, so it was safe to use. But, with that being said, it's possible that your child may be reacting to some of the exposure to the antibiotic. With the use of the antibiotic, there are two possible causes to consider. First, and most obvious, would be an alteration in his gastric flora (healthy bacteria). Thrush is the most obvious manifestation of altered flora after antibiotic use; however, there could be a small, subclinical amount of gastric flora imbalance and gastric irritation. In addition, certain medications can reduce the sphincter tone in the esophagus. This makes it easier for acid to reflux. Sometimes antibiotics are implicated in the increased ease of gastric reflux.   Although, there is a possibility of GERD, again, I question more of a gastric irritation, given his acute onset of symptoms and your recent use of antibiotics. Although you stopped taking the medication on Friday, it does take the body a little time to get rid of the medication. I suspect your son's symptoms will improve almost as quickly as they began. However, I do recommend he be evaluated by his pediatrician. If his thrush check wasn't recent, since Friday, his provider will likely want to assess for it again. Also, his provider will likely want to do a full physical and may be able to provide a little symptom management, such as Pepcid, if indicated.   Finally, if you have any expressed milk frozen before you were on antibiotics, you may consider using this for a couple of days to see if his symptoms resolve.</p><p> </p><p>I hope this helps. So helpful! THank you. I will accept, I just had a couple of questions. I was on the meds for a full two weeks before he had this issue - could it take that long to affect him? Regarding thrush, our doctor saw him Monday last week and last Saturday and he seemed to be ok. I just started giving him a baby dose of probiotics today - should that help with this if it was the meds? Also, we noticed he broke out in horrible acne right around the time I started my meds. Is it possible that was also a reaction from the medication? It has started to get better since I got off the meds.

Very good questions. Yes, antibiotics, even in adults, are often associated with delayed side effects. It takes time for antibiotics to alter bacteria, so it is possible that he could have some gastric irritation later on. As for the acne, it's less likely to be related, but, since you had a recent staph infection, it would be important to have your provider just take a look. It would be best just to ensure that this is just normal infant acne. I do think that the infant probiotics are a great idea. Many mothers use them for infants with gastric upset, and they do seem to help. I really suspect he'll start feeling better soon, but, with any infant, it's so important to just have the provider see the child in person, especially those preemies.

Thanks! I have had the acne checked by both his doctor and my dermatologist and thankfully it is just acne! We did take him into the doctor after his Friday night episode, I just didn't feel they were helpful with explaining why he might suddenly be having these symptoms now. But if he doesn't improve we will take him in and try to get some medication to help him out. Thanks again!
