Question

Hello,

i have been suffering from pain in the right side of the pelvic region that runs up from the extreme right to the middle at the bottom of my lower abdomen. This pain is since past 8 months but it was very less frequent in the past so i thought it is muscular or because of gas. Since the past two weeks it has got worse. And it is so bad right now that I cant even walk.

I consulted a gynaecologist and a general physician and both didnt see anything on the Ultrasound scans. They are unsure of the cause. It is spasmodic pain that comes and goes, doesnt stay continously but it is sure getting worse day after day.

Hi XXXXXX,

May I ask a couple of questions so I can get a clear picture of what might be going on?

---Did your doctor do any blood tests, and if so, what were they and what were the results?

---Do you have pain that is relieved somewhat by having a bowel movement?

---Do you have food intolerances?

---Do you take any medication? And do you take any over the counter meds for this pain; is it effective?

yes, they only revealed lack of Iron in the blood and hypothyroidism.

It is not releived by bowel movement, of course i feel less heavy when the bowels are clean

---Do you have food intolerances?

not really, if there are, I havent made any observations

---Do you take any medication? And do you take any over the counter meds for this pain; is it effective?

I took antispasmodic, but it didnt help

Answer

Hi XXXXXX,

Your symptoms could be caused by a number of things, some of which should have been seen with the ultrasound. Here is a list of the possibilities that may cause the kind of pain you are describing:
  • Kidney stones: these tend to be recurrent. People usually pass stones small enough to pass through the ureter (tube leading from kidneys to bladder), but they can be excruciatingly painful. Diagnosis is usually made by doning a CT scan, or an ultrasound if the person is pregnant.
  • Colitis: this usually happens on the left, but your colon extends from right to left. If this were the case, you should have other symptoms like fever, maybe nausea/vomiting. It can cause very severe pain, and may be difficult to pick up on ultrasound.
  • Irritable bowel syndrome: usually this is more widespread over the abdomen, but some people have problems on one side or the other. The pain involved is abdominal cramping, along with gas, bloating and changes in bowel movements between constipation and diarrhea. However, it should not cause severe pain as you describe. This diagnosis might be made after all other possibilites are ruled out.
  • Endometriosis: this is usually seen with ultrasound, but may require a surgical procedure (laparoscopy) to diagnose. There would probably be other symptoms relating to your menstrual cycle.
  • Salpingitis: this is inflammation of the fallopian tubes. Symptoms appear usually after your period and include pain as you describe plus you may have fever, nausea, and a vaginal discharge. This condition is diagnosed by pelvic exam, blood tests and a mucus swab of vaginal discharge.
There are other conditions causing abdominal pain. I would ask your doctor about kidney stones and salpingitis to see if the testing that was done would rule these out, and if not, maybe do some testing for these particular conditions. These conditions require medical treatment to resolve (as opposed to home care or remedies).
