I am so sorry your cat hurt herself. I do not think she puposely ran away. She just got sidetracked outside and did not or could not get home.
A cats appetite is the best barometer of her health as sick cats don't eat.
She may be hyperthyroid so a T 4 test should be done:

I do not think she is right now a candidate for surgery .She would need to builld herself up first.
I would not make any decisions just yet. Give her some time to rebuild . If she is vomiting it may be her food:

Lack of fluid in the bowel also leads to vomiting, Ingested hair can also be a factor.Anorexia for any extended time leads to liver damage and causes nausea and vomiting as well.
I would add some canned or baby food for her
Serve the two foods separately - do not mix them - add a half pepsid a/c tablet (famatodinne) crushed up into the wet food one time a day.
Cats are attracted to smelly foods - the stronger the better. If she wants tuna, give it to her You can also try fancy feast, clam juice or baby food.
Only you can make life or death decisions for her - I would not rush into anythinng you coud regret later.
Take your time and see how she does. You can always make a decision if she stops eating or worsens.
In the meantime she needs antibiotic so her wound does not get infected.
