I dont understand what you mean by "the middle one"?
And where do you think the infection is?
I do not know where the infection is. I am not a doctor and unfortunately our vets here do not have the appropiate equipment to see where the infection is. They mentioned the MRI but nobody does this.
As blood is coming out from his ear and two pieces of tissue came out, i believe the infection is somewhere deep inside.
You should tell me, you are experts.

I know it can be frustrating working as we do online. I am sorry for the confustion but I could not tell that you meant the middle EAR.
From your description it sounds more like involvement in the inner ear. This is deeper and more likely to cause the bleeding you are talking about. I am assuming that the bacterial tests done by your local veterinarians was negative, showed no bacteria. Otherwise they would not be talking about the possibility of cancer. If the "piece of meat'" that dropped from his ear is in fact tissue than cancer is most likely the correct diagnosis.
An MRI is a very advanced piece of equipment. This will help locate exactly where the problem is located, and assess the situation. IF there is a tumor you may find that it is in an area where the cancer can be treated with laser removal or even a scalper. The test may verify that the lump on the neck is the problem and can be removed.
Not being able to see your at and preform a physical examination, all I can offer are advice and suggestions for you and your local vet.
If it is at all possible I do believe the best next step is the MRI.

I also thought of infection in the inner ear.
The bacterial test showed that yeast, bacterial microorganisms and monilia are present.
We treated him for 10 days with one of the antibiotics written on the lab paper from the test and the only thing that happend is to reduce the bleeding. It seems he walks better (his balance is better). I am afraid to give him every day antibiotics for very long time or should i?
He lost too much weight altough he is eating.
There is no way to perform surgery in Cyprus, nobody does this.
Tell me please, what this tissues might be and how dangerious is that they are coming out. I mean what this means? Is there any way to stop this bleeding? Can you recommend any drops or tablets or whatever for a treatment of the inner ear.
About the lump, the vets say that it a gland that is swollen due to the infection.

These pieces started to come out once we started to use Deroxyl drops (ingredients - Gentamicin, Thiabendazole, Dexamethasone).

Without physically examining the tissue, I cannot tell what it is. If the 10 days of antibiotics helped you were on the right tract. Most antibiotics are safe to be given daily for up to 20 days. I may be that you started to get rid of the infection but stopped before it was completely gone. If a small amount of the bacteria was left, it would quickly regrow once you stopped the treatment. My thoughts are to restart the antibiotics that he was on and continue them for a longer time. If you can get the infection completely cleared the ear tissue should heal. He may always have scars..
The ear drops that you are using may have helped flush the tissue out of his hear. But these drops most likely did not cause the problem. The tissue may have been blocking the ear canal and now that it is gone the medicated drops can better reach the area.
Ear infections is cats can be very frustrating. The outer ear canal is long and curved.This makes it harder to treat and makes the inner ear harder to see on an examination and harder to treat.

My mother told me that she stopped the drops from today and she started with an oral antibiotics again. From the time we started to use these drops he feels pain when we touch the ear and started to smell (it did not smell before and he was not in pain).So she believes that they worsen his condition. You did not explain what this tissue might be and why you believe that is a reason to believe that it is a cancer.
For the moment we will give him an oral antibiotics and will only clen the ear with betadine and keep it dry.

Treat with both the drops and the oral antibiotics. This will treat the infection from both the inside and the outside. The pain when using the drops is normal due to the infection.
It may hurt for now but it is helping. When you were little and needed medication your mother made you take it if you wanted to or not. The same thing goes here.

I can not say if this tissue is cancer or not. The only way to tell for sure is to have it examined under a microscope by a pathologist. These are specially trained doctor who know exactly what cells to look for in cancer and other diseases.
As to what it might be, from what you have told me about the infection and the way the bleeding stopped during the earlier treatment. It is possible that the tissue you saw was skin that became infected and sluffed off. This can be a normal part of healing. The bad tissue sometimes comes out to make way for the healing tissue.
I am sorry if you think I am not answering your question. But as I hope I explained without actually having the tissue in my hand I can not give you a definite answer.

I have discussed this with a couple of other vets. Due to the type of communication we are using I did not want possibly miss anything. They agree that the cancer is a possibility but first address the infection with the extended antibiotics to see how much improvement occurs.
