Question

I am suffering Gout in my right Index Finger Large joint, and in my thumb large joint just above
my wrist. I was prescribed prednisone (a burst of 6,5,4,3,2,1) and 1 I take tomorrow, as well
as colchicine. I have had this for two weeks, and was delayed in getting it checked so it was
a severe attack. Although the swelling has gone down considerably, both joints are very misshapen and still hurt a lot. My main questions are what is it, why, and how did it happen?
Any advice you can give me would be much appreciated.

I have been prescribed a burst of prednisolone 6,5,4,3,2,1 (tomorrow is the last pill). Also I have
taken Colchicine .6 mg. I finish taking that in 5 days.
I do not smoked or drink. I do have rheumatoid arthritis
and I also have low thyroid for which I take Synthroid. I had a total hip replacement in 1998, as well as breast
cancer, for which I had chemo for 6 months and radiation for 6 weeks. Gout has been an entirely new experience and an EXTREMELY painful one too!!

Hi XXXXXXXXXX,

Gout is a type of arthritis that is caused by too much uric acid in your blood. This can form crystals inside the joints that cause an attack of inflammation with pain, swelling and redness.

The treatment you are receiving is the accepted treatment and should be effective. You can also avoid eating meat every day, alcohol (especially beer) and sea food. You can take over-the-counter anti-inflammatory medicines like ibuprofen (Advil, Motrin) unless you can't tolerate this for some reason. But do not take aspirin.

Continue to follow-up with your doctor for this problem, to prevent future and more severe attacks. You might need to take the colchicine for the rest of your life.

Yes I have had blood tests, but my uric acid level was 4.6, which
makes me a little confused. I have never had anything like this before. The swelling and redness has definitely improved, but the pain is still excruciating at times, because I do everything using my
right hand!

Hi XXXXXXXXXX,
It certainly sounds like you have had your share of problems. This problem can come on for no apparent reason. Though there are certain things that make you more prone to an attack, like being overweight, or eating foods that contain "purine" which will cause an elevation in the uric acid. Some diuretic medications might affect it also.

Continue the treatment prescribed by your doctor and make sure to follow-up with him to prevent recurrences. Try to rest the affected joints as much as you can, and elevate your hand when it's painful.

Your rheumatoid arthritis will complicate things, too.

Valerie - I am certainly not overweight - I am 5'4" and weigh 137lbs.
I have found that using an ice back has really helped sooth the pain -I just hope that is all right. I did not ask my doctor about doing that. I also take a diurectic daily - Dyazide 37.5/25. I asked my Dr. about taking Dyazide, and he said that should not cause gout. I am certainly taking the medications as directed and plan to visit my
primary care dr. on Tuesday afternoon. I have been a patient of his for 25 years, so he knows a lot about me and my system! I have even given him something to think about!!! I just have to be patient, and I am really finding that hard this time.

The ice packs are certainly fine.

When you see your doctor, you might ask him if this is really gout because your uric acid level looks like it was within normal range. This could have been a flare-up related to your Rheumatoid arthritis. You would need to know if it truly was gout arthritis because it would make a difference with foods you can eat or not.

Just a thought. It looks like you are doing a wonderful job taking care of yourself. Keep up the good work.

Valerie - Thank you for your response. My rheumatologist seemed to be
sure it was gout. However, I am not sure about my internist. He told me after I have taken Colchicine and if it has an affect on my
hand, we will know it is gout. However, I will certainly ask him on
Tuesday about whether it is, in fact, gout or an RA flare-up! My husband has been deceased for 9-1/2 years, but he always used to say
that "getting old certainly is not for sissies"! I agree with him
1000%!! I have been very fortunate and am now an 11 year survivor from my cancer. A month ago, I stopped my final cancer drug "Femara".
So now my body is entirely fending for itself. I just hope and pray
it continues as well as it has been doing! I try not to dwell on the
fact that I have known two people who did fine after BC for 12 years, and then it came back and they were gone in less than 6 mos. They were quite a bit younger tham I am so - I just have to keep my mind off it and see what happens!
