Question

Hello,

My issues are complex however I see my question as being rather simple. I've been diagnosed with OCD, Bipolar I with depressive episodes and social anxiety disorder. I've since been put on quite a regiment of medications that has built up over about 4-5 years. I will give you a quick rundown on the Medication :

Clomipramine - 250 mg qd

Lithium - 1200 mg (currently cutting back 50% which is a concern of mine. Could things go into a free fall from relative stability?)

Diazepam - 20 mg tid

Lexapro - 20mg qam

Synthroid - 50 mcg qam

Effexor XR - 300 mg qd

Dexedrine - 30 mg spansules qam

Dexedrine - 10 mg tab bid

Lamotrigine - 100 mg qd

Seroquel - 50 mg qn

I have experienced extreme tremors due to medications, and I have tried both primidone and propranolol and they have been ineffective. Things are sort of OK now and I don't want to nosedive by cutting my primary mood stabilizer. Are there any other medications, perhaps unconventional that could be used for tremor?

Already Tried:
Meds in main message. I've tried several other SSRI and benzodiazepines. Things aren't perfect now, but somewhat manageable without hospitalization. I don't wish to lose that. The tremor is incapacitating and I know that I have already tried the two conventional medications. My Doctors are unsure of how to proceed. They've talked me into trying to cut my lithium in half, however I am afraid of the possibilities. Your help is appreciated.

Well, although you have been diagnosed with 3 psychiatric conditions , such as Bipolar disorder , OCD , and Social anxety disorder , but your current treatment regime entails a lot many drugs , some of which are unnecessary and most probably your present symptoms of Tremor are a result of overloading with too many medications , that is Iatrogenic ( drug induced )Tremors.

 

You see you have been currently placed on all 3 types of Anti depressants such as TCA anti depressants ( clomipramine ) , SSRI s ( lexapro ) and SNRI s ( effexor ) , and such type of Poly pharmacy , not only will make you resistant to all the anti depressants , thereby leaving very few or no option for treating your depression , but also it puts you at a risk of a life threatening condition , which occurs due to the overlaoding with too many anti depressants , which is called as Serotonin syndrome .

 

Also, you have been placed on 2 mood stabilizers such as Lithium and Lamotrigine, which i feel is not required , as Lithium can very well control your mood symptoms of bipolar disorder and prevent occurrence of a manic episode.

 

So, i will suggest that you ask your doctor to review the current treatment regime , as it is due to the excessive medications , that you have developed tremors , and this may not limit to tremors and may lead to other distressing side effects also.

 

As far as the drug which may help you in controlling your tremors is called as Requip ( ropinirole ) . Requip , is basically FDA approved for treating Restless leg syndrome ( RLS ) , but is also very effectively used in treating Parkinson's symptoms , in which tremors is one of the symtpoms .

 

Another drug option for controlling your tremors , is use of Anti cholinergic drugs .These may also control tremors.

 

So, you have got two new drug options for controlling your tremors , namely Requip and Anti cholinergic drug , but i will suggest that you give Requip the first try , as although it is unconvental for its use , but is more likely to help in controlling your tremors.

 

So, below i am mentioning a revised Treatment regime , in which i have edited some drugs which are just overloading your system , causing more harm than good , whie their action is well performed by another drug in the regime.

 

1) Clomipramine -- continue in present dosage , it is the drug of choice for treating OCD , and is also very effective in treating depression.

 

2) Lithium -- continue in present dosage ,as i am suggesting discontinuing of lamotrigine , as lithium alone can be sufficient tot take care of your present symptoms.

 

3) Diazepam -- continue in present dosage , this is a very good drug to be used to control tremors.

 

4) Synthroid -- continue in present dosage.

 

5) Dexedrine -- continue in both spansule and tab forms , in the prescribed dosage.

 

6) Seroquel -- as of now continue in present dosage , but you may ask your doctor to slowly bring you off this drug , as once stabilized on lithium , you may not require such an anti psychotic .

 

7) Requip / Anti cholinergic drug -- i will suggest you ask your doctor to start you on requip first , and decide on its dose and time of intake. Anti cholinergic drug is a second option.

 

So, i suggest you to discuss this revised treatment regime with your doctor , and get started on it , and hopefully your tremors will be taken care of , and you will eventually get a relief from them. I have suggested to stop 3 drugs form your current regime-- lexapro , effexor and lamotrigine ,as they are just overloading the system and their actions are well performed by other drugs.

Hello,

I appreciate your very detailed and comprehensive response. It is exactly the kind of second opinion I was looking to get. I do however have a fairly significant question regarding the Anti-Depressants. I'm uncertain that the tri-cyclic medication is effective enough for the depression on it's own, however I can see the Serotonin overload. It certainly is effective in controlling my OCD. I have a history of severe depressive episodes, so would it be acceptable to drop the SSRI and keep the SNRI Effexor onboard because of it's added norepinephrine effects? Also, I'd like to try the Requip however I'm a bit nervous about it's sleep related side effects. I read in the monograph that it can bring about something that to me sounds similar to narcolepsy. Are severe sleep issues overly common when using this medication or would that be only in an extreme case? Thank you.

Well, yes , you may continue on Effexor XR , but , just get its dose reviewed by your doctor , it will be better if you can be sustained on half the dose of what you are taking as of now .

 

Now , as far as Requip is concerned , well the side effect of requip of excessive daytime sleepiness/narcolepsy , that is bothering you , is not commonly seen , plus You are already placed on Dexedrine ,a psychostimulant and that too in both spansule and tab form , and in adquate dose , which itself treats narcolepsy / excessive sleepiness , so if at all such a side effect of excessive sleepiness is produced , then it will be taken care of by dexedrine in the first place ,and you will not be bearing it .

 

So, relax and try Requip ,and also you may continue on Effexor XR , just get its dose reviewed.
