Question

I am mostly wheelchair bound due to a bad case of Guillain-Barre Syndrome 12 yrs ago. I am 47 yr old male who always has had issues with fungal infections since I was small child. Nystatin always seemed to work well up until a month ago after taking Keflex for a drug resistant sinus infection. I have a rash on my buttocks that is not responding to Nystatin. Other anti-fungals do not work which leads me to believe that it is not a fungal issue. I have some relief from Extra-Strength Benadryl creme. It is uncomfortable and was wondering what else could be used for relief. The only cortisone I have is Proctosol. Thank you!

Hi,

It could be eczema, sweat dermatitis, allergic reactions or urticaria.

You need to wash the areas several times with fresh water. Do not use any cosmetic products at the sites.

Apply calamine lotion at the site of the lesions and see if it helps. You could take some oral antihistamine medications like cetrizine or loratadine. You need to maintain a good personal hygiene .

Anti-itch drugs, often antihistamine, may reduce the itch during a flare up of eczema, and the reduced scratching in turn reduces damage and irritation to the skin.

For mild-moderate eczema a weak steroid may be used (e.g. hydrocortisone or desonide), whilst more severe cases require a higher-potency steroid (e.g. clobetasol propionate, fluocinonide).

Eczema can be exacerbated by dryness of the skin. Moisturizing is one of the most important self-care treatments for sufferers of eczema. Keeping the affected area moistened can promote skin healing and relief of symptoms.

I should have mentioned Calamine lotion was tried - sorry for not mentioning that. I do practice good hygiene to avoid bed sores and the like. There is no itching so I doubt it would be scabies. I think it is more uncomfortable while sitting rather than itchy. I guess my last question is can prolonged use of Nystatin with and Triamcinolone Acetonide Cream cause the irritation and perhaps the further use of stronger cortisone cremes would be detrimemntal to the area?

Hi,

Nystatin use should not cause the above symptoms. But yes triamcinolone could cause your symptoms, even prolonged sitting or lying down could.

Glad to know about the perfect care you are taking to avoid bed sores - it is very essential.

If calamine has been tried - i would suggest that you continue using it. Avoid steroids for some time.

Take oral antihistamine medications and see if it helps wit your symptoms. You could keep the area well moisturised.
