Question

We've got a 9 yr 3 mo old Newfoundland male who has been diagnosed with megaesophagus (also has chronic arthritis/badly scarred knees). We rescued him at age 5.5. For 3 days metaclopromide worked like magic. He then began heaving up white frothy mucus again and had trouble holding down food. He has been extremely lathargic. Last night, he received 3 injections at the ER: 1 Pepcid, 1 Rimadyl, 1 antiemetic. One of these really hurt him and his leg has been extremely sore. He cries when he gets up. We have a bottle of tramadol that he was prescribed, but we've never used it. Is this something we could try for pain relief? Our fear is that the side effects include nausea/sedation/suppressing a cough, which would not be good for his megaesophagus. Also, is tramadol OK with antiemetics? Any help would be greatly appreciated!

Optional Information

Age: 9; Male; Breed: Newfoundland dog

Already Tried:
Currently on:

Pepcid injection (24 hour; good until tonight at midnight)
Antiemetic injection (24 hour; good until tonight at midnight)
Rimadyl injection lasted 12 hours, but we crushed a rimadyl and gave it to him orally (good until tonight at midnight)

***Prescription for METACLOPROMIDE - Should we try this around midnight tonight? It worked wonders for a few days - should we go back to it tonight?

Accepted Answer

Rimadyl is a pain med and combining it with other pain meds is best only done if your vet agrees. You can call the ER clinic and see if they will advise using the tramadol but only the treating vet or your vet can tell you if this will be safe for your dog.

If you can find the spot on the leg that is sore you can try a cold pack on it (5 minutes on 15 off) to see if that will help him with pain and inflammation. Typically you use a towel over the cold pack so no tissue freezes.

Cold can work really well for pain issues like this and it won't have side effects like a med could. It may be enough to keep him comfortable until you can talk to your own vet about his problems.

If he had injections in a rear leg there is a nerve that runs down the leg that usually they are very careful to not hit with an injection but its possible it got irritated anyway if the injection produced some inflammation.

and rimadyl here

scroll down the pages for all the info on each med.

Hope this helps you!

Hi! I guess our question focuses more on the side effects of tramadol and its interactions with the drugs other than rimadyl. We've been told that tramadol+rimadyl is ok for him, but we're worried about tramadol+metaclopromide/pepcid and tramadol with megaesophagus (suppressing his cough and making him too sedated)...any experience/research on those interactions by any chance?

And do you think going back to metaclopromide is something to try for tonight if he can take the pill?

Thanks for your help!

Answer

The problem is it is illegal for anyone to prescribe meds or a treatment plan for a dog that is not their patient.

As you know when the meds run out that may help your decision.

You can just call the e-clinic you went to and ask as they have seen your dog.

Pepcid is typically given with pain meds to help keep the stomach from trauma from the meds. You can read about pepcid here

http://www.petplace.com/drug-library/famotidine-pepcid/page1.aspx

and about reglan (Metoclopramide) here

http://www.petplace.com/drug-library/metoclopramide-hcl-reglan/page1.aspx

to see what they do.

I can give you the info for you to make a decision but your best bet for explicit directions is to call the clinic you had him at earlier this weekend.
