i work in mozambique in africa returning to uk i find the soles of my feet have thickened under the balls of my feet they feel a bit like pins and needles and a bit numb. I am a fairly fit person 49 years old very active but my energy level is down, ifeel a bit trembly and a bit sick in my stomach when I wake up and very tired at times , i have taken all my malerials. Can you help?

i have been to the doctors and they said it was probally thickening of the skin due to not wearing shoes around the house . also i have had a course of anti biotics since i came home due to an infection on my lip

Hello thank you for getting back to me, i finnished my anti malerials a week after returning to uk as recomended. i have been on malerone

I have had the symptoms since dec last year (2008) about four months now

no investigations done yet, i have been to the doctor but i gues they are not familiar with tropical disease over here. its the thickening of the soles of my feet i`m especially concerned about

A thickening of skin of feet does not point to any tropical disease and this can be just what your doctor mentioned as skin thickening termed as corn and calluses and are absolutely not worrying. Calluses and corns are among the most frequent skin conditions and, very commonly situated on the feet. Calluses are a diffuse thickening of the outermost layer of the skin, the stratum corneum, in response to repeated friction or pressure. Corns develop similarly, but differ by having a central "core'' that is overgrowth of the skin layers and often painful. Corns typically occur at pressure points secondary to ill-fitting shoes, an underlying bony spur, or an abnormal walking posture (gait). These lesions are typically located on the plantar aspect (sole) of prominent metatarsals, between toe clefts, or on the dorsal aspect of toe joints.

Having mentioned about the skin thickening, ideally any symptoms which are persisting for four months (though they do not look significant) would need routine blood work and metabolic panel. This should give the guideline for the further investigations and management.
