Sorry, I am going to opt out so perhaps someone else can answer, I could not locate a sample and the Court's website http://www.mass.gov/courts/courtsandjudges/courts/appealscourt/index.html did not have a form either. If the court has such a form, a sample may not be available online, I think your best bet is to call the court clerk: http://www.mass.gov/courts/courtsandjudges/courts/appealscourt/courtadmin.html
