Question

My cat, a 12 yr old Main Coon, has a protuberance on the edge of his ear. It seems to be filled with blood, but does not bother him. Could this be from a tick? What could cause it, and how should treatment go?

Answer

Hi XXXXXXXXXX,

This sounds like an aural hematoma. It is a collection of blood within a sac that is usually the result of trauma to the ear. Most commonly the trauma comes from the cat shaking its head or itching its ears repeatedly (they often have an ear infection or a mass in that ear canal).

For treatment, the first concern is to try to determine why the hematoma is present. This usually means a close examination in the ear canal and thorough check of your cats skin. The treatment for the hematoma itself will depend some on its size. If it is quite small, your vet may choose to just let it resolve on it own. If is it larger, than it will need to be surgically incised, drain and then the skin will need to be sutured in a special way to allow the wound to continue to drain, but encourage the two layers of tissue to heal smoothly. This means that a number of sutures will be placed in the ear.

Please let me know if I can be of further help.

It is just on the edge of his ear, the thinnest side, or outside, there is no visible problems with his ear otherwise. We squeezed and drained it some time ago, but it persists.

Do you notice him shaking his head, pawing at the ear, holding his head at an angle, or having any balance problems?

None whatsoever....this is like a pimple or bulge...on the very outer edge of the ear, nothing inside, no scratching problems.

Accepted Answer

I am glad to hear that he is not showing any symptoms of an inner ear or skin problem. This may just be a cyst. The safest option would be to have a vet a take a look at it, or you can choose to simply monitor it at home.

As cats age, some are more prone to developing these cysts, they will often fill with fluid and then either rupture or the fluid will subside on its own. However the sac of the cyst remains, so it will often refill in little time. If it is a cyst and it is not bothering your cat, I would leave it alone. Repeatedly popping it often leads to infections.
