This sounds like a frustrating problem. While I don't know what it is, I can tell you a few things that I would do to try to help if this were my patient.
I would consider doing a skin scraping to look for demodex mites. These are not common, but when they do appear they can cause lesions on the feet that do not improve even with steroid injections. A skin scraping is not expensive and is quite easy for your vet to do.
If that doesn't reveal anything then I would advise doing a biopsy of the area. This may be a little costly, but should give you an answer as to whether this is something treatable, and how to treat it.
