I am a 33 yr old woman who has had cronic migraines for 17 years and also a complex anxiety disorder diagnosed in 1996. I am considered disabled due to those problems and now have been diagnosed with fibromialgia and live life with daily chronic pain that in the last few months has been so bad that I can barely get out of bed most days. I have two children ages 11 and 15 and my life with them is affected also as I am limited as to my quality of life. I am now on lyrica fluoxetene cyclobenzaprine alprazolam and atenolol and recently my doctor took me off of 40mg oxycontin because of stomache and itching problems and put me on short acting oxcodone. I am allowed 40 tablets per month and it isn't enough, it doesn't even take the edge off. I have spoke with him and he is resistant to perscribing me the suggestion of methadone by the pharmacist. What do I do?

I think the most logical thing is to
1) See a pain specialist
2) consider an alternative treatment for your fibromyalgia
First, with regard to the pain medications: Oxycodone is the active ingredient in oxycontin, so if your doctor is concerned about an allergy, he has given you the same medication in a different form. If you were on oxycontin 40mg/day and you have been dropped to 40 tablets of Oxycodone 5mg (or 10mg)/month, that is a
huge
drop. I do not know the specific issues of why this change was made, but I think you need to discuss this further with your doctor.
The pharmacist's suggestion is a good one, but many physicians are very wary of prescribing methadone for chronic pain. I think it is actually a very good choice as long as it is managed just as one would do with oxycontin. Seeing a pain specialist would help in this area.
In terms of the fibromyalgia, be sure the lyrica is at an optimal dose. If it is and it is not effective, you should consider asking your doctor to change you to Cymbalta, tapering the lyrica and fluoxetine. For the overlap between meds, remeron (a different type of anti-depressant) might help you out, particularly if you have trouble sleeping.
Lastly, I have had patients tell me they have achieved success with an alternative approach to fibromyalgia and chronic fatigue syndrome from using Dr.. Jacob Teitelbaum's program, though I do not know enough about it to endorse it. I am giving you the link because you seem as if you've tried most everything else.
