Question

Licked all the hair off her belly. Scratches frequently. Does not have fleas. Drinks alot of water (always has). Taken to vet who gave her cortisone. Cat had a reaction of lethargy and did not wash for weeks. Blood work during time of lethargy shows only slightly elevated blood sugar. Vet states no diabetes. Hair started growing back during time of lethargy and no washing. Has since licked it off again. Otherwise, happy cat. I'm nervous to take back to vet due to trauma of cortisone injection and the visit for the cat.

Thanks for the question XXXXXXXXXX.

This is actually a very easy question for me as I have seen a great many cats like this in my consulting room. The pattern of hair loss will be confined to the stomach and perhaps the inner aspects of the thighs as well. The most likely diagnosis is allergic skin disease of the cat.

There are a number of possibilities as to the allergen [ the cause of the allergy ]

1. It can be related to flea bites, the flea injects a small protein in to your cat's saliva when it feeds. If your cat has a predisposition then only one or two flea bites will set it off.

2. It can be related to other small proteins which get into the cat's blood stream via inhalation. Pollen and house dust are often implicated here.

3. Your cat could be allergic to something in its diet.

Often it is difficult to work out precisely which allergen is causing the problem so these cats often get put on anti-allergy medication by your vet and this works well but may not be a permanent cure, cortisone is often used but it is possible to adjust the dose rate and there are other drugs as well.

It is possible to carry out allergy testing and I have done this a few times but the results can be unreliable. Of course you should be meticulous about flea control both on your cat and the house as flea bites perhaps even only a very few are often related to this condition.

Hi Dr. Scott:

 

About the fleas - we have two cats - and the other cat does not have this problem. I do not see any flea dirt in the cat's fur. However, they cats do go outside in the backyard on leashes. Do you think trying flea medication (on back of neck/shoulder area) may be a start to try to rule out an issue.

 

The cat's wet food has been switched frequently (because my other cat is finicky). However, they have had the same dry food for years. This condition appears to have only started several months ago (or I just recognized it then!). Is there any canned food you may recommend for a cat with this issue? The current wet food is Solid Gold/Blended Tuna.

 

I'm nervous about further Cortisone injections, although the vet also stated there were other shorter lasting Cortisone injections available.

Glad to advise you ....

1. Only a few cats are hypersensitive to flea bites, perhaps one in a hundred. The rest of the cat population can have loads of fleas and be just fine. The hypersensitive ones only need a couple of bites and you can get a massive skin reaction.

2. It would always be a good start to thoroughly control fleas when you are fighting skin disease in the cat.

3. There are hypo-allergenic prescription diets for cats from companies such as Hills, your vet will advise.

4. It is worth talking the cortisone reaction over with your vet and expressing your concerns, there are other ways of using this drug and different dose rates. I generally found if I worked with the client we could sort something out.
