The most common cause of swollen glands is an infection, this may be bacterial or viral. If the paracetamol is not bringing down your temperature, or you are having problems swallowing then you will have to see the out of hours doctor by ringing NHS Direct or NHS 24 depending on your location.
An over the counter gargle (Difflam) can help with the inflammation but not the underlying problem and I do feel that this needs examination to rule out a quinsy or abscess.
