Hello and thank you for your question.

This crib should have a warning label on the inside of the headboard that has a date of manufacture (month and year) as well as a model number.
The model number may be the number you already provided, though most Gerry/Evenflo cribs had a 4-digit number.
Could you check and see if there is a month and year, and/or any other number on this sticker?

I need to add that there were at least 28 different model numbers of these cribs manufactured between 1991 and 2000 that were RECALLED because they have a defective design that allows the mattress and bottom of the crib to fall to the floor.

There was an upgrade kit supplied for these cribs to fix this problem.

The Gerry Baby Products company has since gone out of business and was bought by Evenflo.
I would recommend that if it's impossible to determine from the date and model number that this is NOT one of the recalled models that you contact Evenflo at the "Evenflo ParentLink Department" at 1-800-233-5921 and see if they can establish that this is a safe model, or help you in obtaining replacement parts otherwise.
I don't think that you should attempt to assemble and use this crib until that is determined.
