Guardian ad litem

My jurisdiction is: Wisconsin Brown County

Hello, My ex and I have just went to mediation for custody issues of my 12 year old daughter. We were unable to come to an agreement and our mediation reached an impasse. I recieved a letter stating that" upon written request of either party, a guardian ad litem shall be appointed without further hearing". I just need a template or sample letter to send because I have no idea what to write. Once the letter is written, to whom do I send. Thank You.

Pursuant to the courts request we have completed mediation in the above referenced cause and have been unable to resolve the custody issues. I am requesting that the court appoint a guardian ad litem and proceed to set the matter for hearing on the merits. If anything else is needed here please advise in this matter. I appreciate your attention to this matter.
