Question

Hello, My 27 year old daughter just went through having two blood clots in here lungs so they put her on coumadin she has not been doing very well at all since she has been on this medication, she is extremely tired and wakes in the night with having almost seizure like episodes she can't walk and has lots of pain in her legs she become very very sick feeling this last for about fifteen to 30 min she was on lovienox shots for 2-3 weeks after her hospital stay. I am very worried she is not the same young women she never complains and works very hard she is on her feet 8hrs working as a cocktail server she also was on the birth control pill yaz and has know gone off what do you think and thank you verymuch

I would like to get a little more background info before I try to answer your question.

Did they diagnose your daughter with anything other than having pulmonary emboli, such as DVT (Deep Vein Thrombosis)?

What coagulation testing did they do? Did they do any special coagulation testing for Protein C, Protein S, or Prothrombin gene variant? If so, what were the test results?

Also, is her coumadin therapy being monitored carefully, and is she taking her coumadin as prescribed, and watching her diet since certain foods, such as green leafy vegatables, can counteract the effects of coumadin?

At age 27 it is very unusual to have a pulmonary embolus. When people this young have clotting problems it is usually the result of some genetic deficiency in one of the clotting proteins. Any more information you can provide will help me answer your questions.

If your daughter has not had these tests performed, then I suggest she see a Hematologist ASAP to have this testing done, because another pulmonary embolus could be fatal.

hi lance, thank you for getting back with us. we have some of my daughters labs here i am going to copy some info. from them and mabey you can explain some things to us.

test requested; LUPUS ANTI. REFLEX;cbc with differential/platlet; comp. metabolic panel (14) prothrombintime; thyroxne(t4) free, direct, s; tsa; ana w/ reflex; venipuncture.

interpretation; no lupus anti was detected. an extended drv vt that corrects on mixing with normal plasma can be caused by a deficiency of one of the common pathway factors ( X, V, II, of fibrinogen). my last dr v vt was 49.3 my prothrombin time as of 11/24 was 15.3 and i think my inr was 1.5 not sure what the difference between these is and my platlet count was 440 and my alt (SGPT) was 42. these are just some of the highlighted numbers in my lab tests. i have been on coumadin for a month now but feel like my body is like an elderly persons. my leg and arm muscles have no strengh at all i used to go to the gym 3 times a week weights and running my body wont let me do any those things at the gym right now. thank u for your time.

Hi Love, Your test results do not shed much light on your situation. Your INR might be a little bit low for your condition, but it's difficult to say on only 1 test result. Your other results, although slightly abnormal, do not point to anything in particular. The factor deficiency indicated is not likely the cause of your problems since factor deficiencies generally result in bleeding problems and not clotting problems. I still believe that your problem most likely lies in a deficiency of Protein C or Protein S, or the presence of the Prothrombin gene mutation. I'm curious to know if anyone in your immediate family has or had a history of thrmbosis or bleeding disorders. The muscle weakness I cannot explain except that it is probably not related to your thrmobotic condition. I am glad that you have discontinued the birth control pills, however your occupation may be a contributing factor to the clots forming, especially in your legs causing the leg pain. Your other symptoms are very vague, so it would be difficult to relate them to any particular diagnosis. I believe still that you need to be fully evaluated by a Hematologist to determine the exact cause of the emboli, and a generall assessment of your overall health to determine if there is some secondary condition going on that is causing your other symptoms.
