Question

My fiance is an alcoholic who was in recovery, but has now returned to drinking. He has psychotic behaviors after several drinks which include repetitive speech, believing he is in imminent danger, believing he is being accused of crimes he didn't commit, being in fear for his life, thinking I am another person, etc. This mainly occurs with the use of hard liquor, but sometimes with a quantity of beer. He walks very stiffly with a wide gait. His pupils become pinpoints and his eyes have a fixed forward stare. He sometimes uses made up words and is very, very paranoid. He never remembers any of it after the fact. Is this just being a drunk or is this a greater problem? Where can he seek help?

I've done some research on the internet, strongly suggested he get clean and sober, separated from him until something is resolved, and prayed alot.

Answer

He needs an urgent review by a family doctor and possibly by a psychiatrist. There may be an underlying psychiatric problem which the alcohol is exacerbating.

I already knew he needed to see a doctor. This response did not really answer my question since I am seeking to find out if this, as he says, is just being drunk. I'd like some ideas for specific avenues or possible illnesses so when he sees a doctor they don't send him away because they think he's just an alcoholic.

There are many diagnoses that could be the cause. It is only after assessment by a doctor that a diagnosis could be made or even suggested.

I will ask another doctor to look at this if they can help but he really needs an urgent review.

Possible issues would be good then I'd have a better idea where to send him. He's currently working in a small town out of state and I know a local GP won't have a clue what to do with him.

I would then recommend seeing a psychiatrist directly in the local city for a review....another option is to see a psychologist who if worried can contact a psychiatrist urgently.

In general, I don't usually need to spend money to have someone tell me to see a doctor. I'm sure this won't be of much help because he won't just go to the doctor for something he doesn't see as a problem and I can't influence him in any way with - "hey you need to see a doctor right away, you might have a problem."
