Already Tried:
Not much, asked a couple of lawyers who didn't seem to know much, did a Google search which didn't help much.

Answer

You would need to have a lawyer do so or go to court and have the court issue and order it for you. Typically done within the discovery process of a litigation.

I realize I need a lawyer to do it, but typically he has a 'server' serve a subpoena on the witness/holder of the info. How does he do that for someone's phone records - and to testify in court (i.e. to use the records in court) we'll need to subpoena a Verizon employee to testify about them. So, to whom do we serve such a subpoena?

Does it really require the court to issue an order? - (most subpoenas don't)

If the lawyer issues the subpoena, the court need not approve it. Now the subpoena can be for documents (duces tecum), for deposition or for both. The subpoena typically asks for a person with knowledge of the information appear if a deposition is asked for. The process server company will handle service for a fee and issue an affidavit of service.
