Already Tried:
She is spayed with a tucked vulva. She licks and is pain / and or discomfort. First we tried Cefa-drops, which I think were making her sick, so I stopped it. Then we did a 10 day series of metronizadole. She was simultaneously on bio-mox because she had a dental with 19 teeth pulled. She seems to be recoverd from the dental, also from a emergency visit to vet for pancreatitis. But the vaginal licking is persistant. I clean once / day on the outside with Nolvasan, and once / day with medicated shampoo for possible yeast, with leave on conditioner "ResiCORT". But I just noticed that she gets very angry when I gently open the vagina and it's VERY red inside. This is an area I have not been cleaning because I didn't know it that would be okay.

It sounds like your girl has a problem with recurring vaginitis and also dermatitis in the skin folds as well.   With skin fold dermatitis, You will want to keep the area clean and dry. Benzoyl Peroxide wipes are also effective for cleaning the folds as well and to keep bacteria in the area down. I see you have already tried miconazole cream to help clear the dermatitis if it is a yeast type dermatitis. Since it sounds like the vaginitis is back, antibiotics are necessary. You need to put her back on them. If you don't have enough for a full course, you need to contact your vet again. Once the infection itself is cleared, keeping the area dry and free of bacteria with the wipes should help keep it cleared. You can read about skin fold dermatitis here:

If your girl is overweight, getting your dog's weight down will also help as being overweight does contribute to the amount of skin folds in the area.

In some cases, no amount of cleaning the area seems to help prevent the infections and dermatitis from reoccurring. In these cases, surgery can be done to reduce the amount of skin fold to help prevent bacteria in the area. You can read about this surgery here:

But is metronizadole the right antibiotic to put her back on ?

Metronizadole can be used or the Cefa-drops, but a full course of the antibiotics are needed.

Are you saying that Cefa drops and Metronizadole are equally effective against a vaginal infection? Also, what's the best way to tell if the infection is only on the outside ? Does it start on the outside and travel inside ?

Susan,

Both can be used for vaginitis. I would prefer to use Cefa drops, but since you felt they caused your dog problems, then Metronizadole could be used. Vaginitis is in the vaginal area of the dog. The resulting vaginal discharge then reaches the outer skin and can cause itching and irritation of the outer skin of the vulva. You can have an irritation to the outer skin (skin fold dermatitis) withou vaginitis being present but they are frequently seen together.
