How do i write up a prayer for relief in pleeding format?

Already Tried:
my cousin was awarded money from superior court. long story short, i had 2 legal cases against her. her attorney sent me an agreement letter. I did all that was asked of me. i dropped the other 2 cases against her. Now he sent me another letter asking me for other things which i will not do. he has totally represented himself frauduently (forgive my spelling)and I need to send the court my proof and ask for a prayer or relief showing that i dropped everything and he is giong back on our agreement. the court said it needs to be in pleeding format. what do I do? I would get a lawyer to help me but I cannot afford one.

wherefore I respectfully requests that the Court [insert what you want the court to do. whatever the other side has failed to do] together with costs, disbursements and such other relief as the court deems just and proper. You may want an attorney to prepare it and represent you and if cost is an issue can ask fora pro bono referal from the bar association or legal aid.

i understand what you are saying. however, i have to have this in by friday. could you tell me where to find an attorney that can prepare this for me? i went onto the website that you suggested but was unable to get anyone.
