Since when is this? Is this the right testicle or left? How big it becomes? How long does it stays swollen? How does the swelling subsides? Is it painful and tender when swollen? Are you diabetic? Any past prolonged illness? Do you have any urinary symptoms? Were any investigations done?

on and off for a year i think, yes it is the right testicale, i had a full medical 8 months ago, i am not diabetic it swells two times bigger than the other not painfull not to sure how it goes away

Hello,

An on and off swelling, which is not painful, can be following;

1) Varicocele; this is swollen venous (pampiniform plexus) plexus, which are multiple veins of the testes.
2) Hernia
3) Hydrocele of the cord
4) Spermatocele
5) Lymph varix

This needs an evaluation by your physician and an ultrasound study.
