"Encephalopathy" - simply means a type of disease affecting the brain.
One of those is "hepatic encephalopathy", which is seen is end-stage liver disease patients, and with high ammonia levels can cause confusion, obtundation, and other physical signs related to dysfunction of the brain.
There are other causes as well:
High blood pressure: hypertensive encephalopathy
Wernicke's: thiamine deficiency in alcoholism
Lyme disease can cause it
Anoxic brain injury, or lack of oxygen for an extended period of time can cause it.
