frankly, filing a writ is way over the head of the average person, and even most attorneys won't do them. There are no judicial counsel forms involved. In my line of work, I file about 3 per year, and I would consider myself an expert among attorneys for doing so. I strongly recommend that you retain an attorney to handle it for you.
If that is truly not an option your best recourse would be to visit your county law library and ask the library for a Bender's or CEB guide on Writs & Appeals. Best of luck.
