Question

Hi all, I am a very concerned boyfriend looking for some help.

My girlfriend (34) had her LMP on 6/17, we were under the assumption that she is lunar and fairly regular.
7/20 - A posisitve EPT
7/24 - A negative EPT
7/25 - Blood drawn with an HCG of 7. (Yes, just 7)
7/26 to 7/28 - Some cramping and bleeding similar to a normal menses.
8/1- A blood draw with an HCG of 56 (Negative ultrasound)
8/4- Blood draw and an HCG of 262 (Still negative ultrasound)

I understand that you need to have an HCG of 2000+ to see anything on an ultrasound.

As you can see, we are very confused, they want her to come back on 8/10. I have been on the net for a good 12 hours researching what the heck is going on and am obviously very concerned for her. And waiting until the tenth is causing me a great amount of consternation.

Can anyone shed light on what might be going on?

Thnk you so much,

Dave

Thank you for your question. It is my privilege to be able to help you.

I want you to know that I logged on and read your question earlier today, and i have been puzzled by it. i am trying to think f the most likely explanation for what is truly going on.

Before I even start with an explanation, I want to reassure you that you do not need to be nervous about it Nothing bad is going on at this time. Your girlfriend will either have a normal pregnacy or she won't, but since she is getting regular surveillance and care, she will be fine, so relax.

Now we can try to take apart this puzzle and see if we can find some answers.

From 8/1 to 8/4 she had her quant rise from 56 to 262, this would be an appropriate rise for the someone who is around three or four weeks or so pregnant, and it is encouraging. This would correspond to a conception date somewhere around July 16th. Now since the quants are not a good way to evaluate dates, she may have had a conception date anywhere between the 10th or so and the 26th or so.

The bleeding from the 26th to the 28th is the truly confusing thing, however, we can conclude with certainty that it was not a period, because if it was, how could her quant be 56 just a few days later. so we must conclude that it was a threatened miscarriage, and not a period. Since it was a threatened miscarriage, it is possible for the pregnancy to continue to develop, or it is possible that it will be lost.

So we've established two things, a conception date between 7/10 and 7/26, a threatened miscarriage between the 26th and 28th, and what seems to be a developing pregnancy on 8/1 through 8/4 and presumably still developing.

So lets assume that the conception was early in our acceptable range, around 7/10, then she would have had a threatened miscarriage, on the 26th, but if she kept the pregnancy, the quants would be expected to be exactly what they are.

Hold on! Where getting there! The only thing still left to explain is why on the 20th her HPT was pos, and on the 24th it was neg. well, now it makes sense, because if she conceived on the 10th, the level of hormone for the first two weeks are very low, and they sometimes may be detectable, and sometimes not. This would depend on the time of day, how dehydrated she was (how concentrated the urine was) and many other factors. so on one day it could be positive, and the next day negative, and than the blood test would show a very low level as well.

Got it?

Conception 7/10

First weak positive UPT - 7/20

First negative UPT because of (very low hcg levels) - 7/24

Very low blood HCG of 7 - 7/25

threatened miscarriage - 7/26

Pregnancy makes it past the "threat" - 8/1-8/4

Now one of two things may happen - she could have a happy healthy pregnancy, or the quants could go down and she could lose the pregnancy. i hope you hear good news!

I hope you find this helpful. If you have any more questions please feel free to ask me. If you are satisfied, please remember to ACCEPT the answer so that I get credit for my work. BONUSES and positive feedback are always appreciated. Good Luck! Dr. SaulJE
