I am sending you a link to the HIPPA guidelines (www.hhs.gov/ocr/hipaa), but wanted to point out a couple of things. First, I think you should report the doctor. You are given 180 days to report.
Second, there are no personal damages allowed by a HIPPA filing. You can consult with an attorney in Denver who specializes in health care law to see if you have a private suit against the doctor. www.denbar.org. Talk with several lawyers and see who you like best. Generally there is no or a minimal fee for the first consultation. You can file the HIPPA complaint on your own without a lawyer.
