Hello,
Thank you for your question. I am happy to assist you.
Please clarify whether your name was changed due to a marriage or divorce, or whether you had filed a petition for a name change.

Generally speaking, you will need to contact the court that issued the order changing your name.
If you have the case number, it will be a simple process. You will want to call the clerk of the court and ask what the fee is for a certified copy of an order. Send the check, a self addressed, stamped envelope to the clerk's office with a letter referencing the case number and requesting a certified copy of the order.
However, if you do not have the case number, you will need to have a name search done at the clerk's office of the court that issued the order. The name change is typically indexed by both the former and new names. To perform your search, you will want to check to see if the name index is on line for that courthouse. If not, you will need to either go to the clerk's office or hire someone to go for you. The clerk can show you how to search. After your search is completed, you can purchase a certified copy of the judgment from the clerk.
I hope that the information which I provided was helpful to you.
Best wishes for a successful outcome. If you have additional questions, please do not hesitate to submit them to me directly.
