There are a number of free pre-made scripts that you can use for this purpose. The one I prefer is Jack's FormMail. There are many others, but this has been the one that I've found most simple to integrate and use. It should include pretty much everything you need in a generic mail form. If it doesn't meet your needs, it can also be used as an example to make another form with. There is also another that I have used here. I found it to be less simple to use than the previous one, but it might provide more in-depth options if you need them.

With either of these, configuration instructions are included.
