It is nice to see you on net again.
As i described you previously the causes for this syndrome are following-
1.It is associated with the design of femoral prosthesis.
2.possteriorly stabilized TKR are more associated with this complication.
3.A small patellar component that fall to lift the quadriceps tendon away from the anterior edge of inter condylar notch( anatomical location in the femur).
4superior placement of patellar button.
5.post operative inflammation also play a role in it.

The recommended treatment for this condition is-
1.Arthroscopic debridement of fibrous nodule in which through a small hole in the knee instrument passed & procedure performed.
2.open synovectomy & possible revision of patellar component.
