If you have a piston slap or rod knocking, it will need a lower end rebuild. It is always a good idea since the head is off to have it sent ot a machine shop and at the very least checked.

If the bottom end(pistons, piston rings, rod bearings, main bearings)has to be rebuilt, the cylinder head will need to be removed. Since the cylinder head is already off the vehicle, it is a good idea to have it sent to a machine shop to make sure it is ok(valves, lifters, etc).
With an engine knock, any driving you do, is basically on borrowed time. There is no way of knowing how many miles you will be able to get before the engine seizes(locks up). When the engine locks up, the damage to the engine will likely increase, as well as any repair bill.
