On most toilets the flush lever/handle and the actuator are 2 separate parts. You may need to replace the lever. What brand toilet is this?

It is Kohler. And it is brand new. What happens is that occasionally the internal Flushmate tank seems to tilt just a bit so that the flush lever misses the actuator arm
