Alright pull it off. It is a cover that will expose a hex screw. You will need an allen wrench. If it does not easily tighten then loosen it and see if the valve body that it attaches to is loose. If you do end up taking the spout off be sure to cover the tub drain with a towel. That way if the screw comes all the way out it does not fall down the drain.
