It's a Kohler but you don't know the model? I have removed the set screw in the bottom of the handle and the handle won't come off

Once the set crew is out the handle should just pull off. If you grab the long side of the lever and move it back and forth toward the faucet it should loosen up. If not the handle may be seized and you might need a handle puller (see attached pic).
