My toilet tank is losing water, once it fills it seems to drain

My toilet tank is losing water, once it fills it seems to drain out somewhere and in order to get water back into the tank i have to manually touch the other part that then pits out the water back into the tank. what is causing this problem and what do i need to do to fix this, please advise and thank you for your help
