hi
ive had a ctc nose scanof sinuses and endoscopy but i keep getting secretions white dry or mucosy but consultant said its showing nothing so why does tip of my nose have small twitching like spasms and its obvious when im with people it starts sometimes also my nose everytime i get this spasm feels like is growing bigger from the tip

does anyone know whats going on pls as im confused ?

Plastic Surgeon : thank you for your question. I did converse with you in the past. The first thing that you should know is that it is excellent that your tests are normal. I have discussed your twitching with a number of my colleagues and the consensus is that this is a possibility after nasal surgery but is usually self- limited (will eventually stop on its own). This twitching may be due to some trauma to the superficial nerves affecting some of the muscles in the nasal tip. Some other causes are medication reaction, anxiety, or allergies. If the twitching is bothersome, an injection of botox will stop this twitching. As far as the secretions are concerned, chronic siusitis either from an infection or allergy is the most common cause. My suggestions would be the following: 1. Consolation with another surgeon who performs a lot of rhinoplasty procedures 2. Consultation with your family doctor and if need be, an allergist.

I did have rhinoplasty about 4 years ago.

After about six months the secretions started.

the twitching just started now about a month ago

This is the 3rd consultant ive been to say.Do you think I SHOULD SUGGEST this time about a possible infection because i dont think from what i recall they did any tests?

When i had rhinoplasty it was to straighten a bump but now even the bump has reappeared

The most worrying thing for me is the twitching which has made the tip grow to the side and im very conscious of it.

I dont want rhinoplasty yet until the secretions stop.Do you think they will now ever stop ?

You mentioned possible allergies and medication.I am on thyroxine 50mg and gilpizide 10mg daily

i have stopped all nasal medication as it was making my nose sore

Is their anything you could recommend which might work?

Regarding botox will 1 treatment just completely relax the spasms - what exactly will it do ?

I never had this problem before surgery.After surgery it started and the secretions fall out when dry to the point i dont go out with friends etc as its afffecting my life.

I really appreciate your answers as they re different to what ive been told.There is obviously something wrong and my nose tip has grown a bit more everytime i have a spasm.

If you have seen 3 different consultants, you really should see your family doctor at this point about possible allergies, anxiety and medication reactions. A referral can then be made to an appropriate specialist if needed. The fact that the hump has recurred and your nose is changing is unusual this long after surgery. Your family doctor can check about any possible infection. A culture of your drainage should be done to see if there are bacteria in your drainage. One botox injection should relieve your twitching but more injections may be needed if the twitching recurs. You should discuss with your family doctor how this is affecting your life.

The botox will deaden the nerves to the muscles of the nose that are causing the twitching and the twitching will stop. You should also refrain from doing anything to the nose - picking or attempting to clean it out unless absolutely necessary. Doing this to the nose repeatedly will cause swelling.

Thank you for your replies.Unfortunately my nose gets blocked with secretions so i do have to pick and clean it out .Ive been told i snore very loudly which is another problem this might have created .

They then dry and fall out without any warning

Does the culture of the drainage hurt? is it a procedure or just a minor test at the gp surgery.

When i take thyroxine with gilpizide i realise i have palpatations during the night and more recently,this might be related or not as i need to see my gp about this but ive had a few episodes of very intense burning sensation frm the bottom of my back up to my shoulders only and then parallel straight across to my chest.The burning sensation is so awful that it leaves me very sore shoulder blades and very sore chest in the following morning.

i didnt know what medicine i could take for this so i hav ent .but it doesnt come all the time just about 4 times this month after i ate i think was the first incident .

do you think their could be a correlation again how could i find out because i mentioned it to my gp who said it could be to do with reflux?

i obviously need to be sure as my back ribs feel very sore to the touch my shoulder blades feel very sore and my chest feels sore so im not convinced.

As im coeliac ,i had endoscopy as i was bloating all the time and my gp said it was too much fibre so having now a low fibre diet has helped the bloatedness has helped so i dont think its reflux.

But im mentioning this to you so you know exactly whats been going on incase you can figure out some correlation between my nose and burning sensation in my back and also ive had bowel spasms few weeks ago which consultant said was irirtable bowel.

What do you think?

The culture of the nasal drainage is simple and not painful. It is done in the GP's office with a QTip. The reflux and bowel symptoms and snoring could all contribute to some of the nasal problem. It sounds like you still may have sone septal deviation and possible sleep apnea. You really should discuss this and your medications and palpitations with your GP,

The septum is the structure in the middle of the nose. A deviated or crooked septum can cause snoring and breathing problems.
