Customer Question

My bone density scan was -4. Have been taking Boniva for several years and large doses of calcium with V-D. Have been advised to begin taking Forteo. Some of the articles I have read are not too favorable--such as bone cancer in rats and you can only take Forteo for 18 months and what happens then. Have been on Lovenox for 2 years as I have a history of blood clots--had one pulmonary embolism. Now I am giving myself Fragmin shots for the blood clots. Would appreciate your input. E. Cox

Drugdad :

Hi, let me help you with this

Drugdad :

It is my understanding, you are changing forteo for boniva, and fragmin for lovenox right?

Drugdad :

Because of safety concerns, forteo is only limited to 2 years of lifetime use

Customer :
Drugdad :

Certain literatures does point out that forteo may increase the risk of osteosarcoma, a secondary malignancy but percentage is quite small

Customer :

I am currently taking Boniva once a month. Have taken Lovenox shots for two years. Have just started Fragmin shots instead of Lovenox. My doctor thinks I should begin taking Forteo shots for my bones.

Drugdad :

both drugs have interaction, it states that concurrent use of both drug can lead to impairs the anabolic activity of forteo in trabecular bone and reduces the beneficial effects of forteo on cortical bone

Customer :

I am interested in Reclast and Prolia. How do they compare to Forteo?

Drugdad :

You may not qualify for prolia because Prolia received FDA approval for osteoporosis prophylaxis in women at high risk for bone fractures after receiving adjuvant aromatase inhibitor therapy

Customer :

What drugs having an interaction are you referring to? And the use of what drugs can lead to impair the anabolic activity of forteo?

Drugdad :

Okay let me clarify

Drugdad :

Boniva impair the activity of forteo

Customer :

Please explain "adjuvant aromatase inhibitor therapy.

Drugdad :

it is a class of drug used to treat breast cancer

Drugdad :

As for Reclast, its advantage is the once a year treatment.

Customer :

Back to forteo. If I begin the forteo shots, I will cease taking the Boniva.

Drugdad :

It makes sense to only use forteo

Drugdad :

Any thing else I can help you with today?

Customer :

Can you give me more info on Reclast. Are there any negatives?

Drugdad :

yes, it does have an extensive list of side effect just like forteo

Drugdad :

I will provide you with most of the serious ones

Drugdad :

Renal impairment, atrial fibrillation, bronchospasm, pantocytopenia, hyperhidrosis, hypotension

Customer :

Back to forteo--what do you take when the 2 years is up that you have been taking forteo?

Drugdad :

Your doctor have to switch you back to a different drug like boniva

Drugdad :

I hope I have answered your questions, but if you have additional information you need to clarify or ask, feel free to ask

Customer :

Of the two forteo and reclast, which would you recommend if you were me? Would if help if you had a list of the drugs I am now taking?

Drugdad :

yes

Customer :

My drugs currently taking:

Customer :
Customer :

Furosemide 40 mg

Customer :

Prednisone 5mg (temporal arderitus)

Propranolol 40mg (for shaking)

Drugdad :

write the drugs you are taking I will take a look at them for you. I only have a few more minutes.

Customer :

Spiriva Handihaler 18mg

Morphine Sul 60mg Fibromyalgia

Drugdad :

If your blood pressure is under control, reclast may be better for you in that regards

Customer :

Vit E

Iron

Customer :

Lutein

Potassium

Customer :

No high blood pressure, surgar, cholesteral normal--just subject to blood clots.

Drugdad :

okay

Customer :

What is the okay regarding?

Drugdad :

Am really sorry but I have to close, if you want, I can get another expert to finish this conversion for you.

Customer :

Are you going to leave me hanging? Do Not Charge Me for this.

Customer :
Customer :

I would like another expert. I don't know any more now than I did when I started.

Customer :
Customer :

Can I finish this tomorrow? It is getting so late and I have to get up early in the morning. Could another pharmacist review my file and give me some info tomorrow?

Customer :

The pharmacist could send me an email in the morning if you do this.

Customer :
Drugdad :

Okay, tomorrow you will get a report from me.

Hi,

Based on the list of all these drugs,
Dalteparin
Furosemide
Morphine
Potassium Salts
Prednisone
Propranolol
Vitamin E
Zoledronic Acid or Teriparatide
Tiotropium
Ocuvite® Lutein

Forteo looks better because it does not interact with any of the drugs but you can only be on it for 2 years. You already know this.

Reclast when combine with furosemide can cause or lead to hypocalcemia, this is definitely what you want with your history of osteoporosis.
