My shih tzu is rubbing and the side of his jaw with the side

my shih tzu is rubbing and the side of his jaw with the side of his front paw, maybe using his 5th toe so, sometimes he screams and shake his back leg , i taped his 5th paw so he couldn't use i ask me vet was it his teeth, he said, he's fine but it has not stopped this started around April / May of this year. I work but do spend as much time with him as possible, he is home with my other dog, both are about 17 18 lb don't know what else to do
