I have a 12 year old Shiba. It started getting black patches

I have a 12 year old Shiba. It started getting black patches that look land feels like cradle cap. Then the hair falls out in no defined pattern. The skin is pink and smooth where the hair falls out. Has it on his stomach and back. Vet gave him antibiotics and put him on steroids. Didn't really stop it.The hair will eventually grow back but the skin seems to get raw.

Hello, I am Dr. RB.. Has your DVM completed blood work to rule out endocrine disease? Many endocrine diseases in dogs can cause patchy alopecia including hypothyroidism and hyperadrenocortism.. Skin infection is usually associated with moist or ulcerative skin lesions, not just the alopecia alone. Another potential cause could be Demodectic mange.. Do you remember if your vet completed a deep skin scrape or skin cytology test?
