This would be predicated upon what that swollen salivary gland indicates. We need to differentiate sialadenitis - inflammation of a salivary gland from sialadenosis - non-inflammatory, nonneoplastic enlargement of a salivary gland from necrotizing sialometaplasia - squamous metaplasia of the salivary gland ducts and lobules with ischemic necrosis of the salivary gland lobules from salivary neoplasia - benign or malignant lesions arising from salivary glandular or ductal tissue.
We first attempt to do the above by needle aspirating the gland and having our pathologist review the aspirate. If results are equivocal we might consider punch biopsying the gland. Removal of the entire gland is rarely performed. Please respond with further questions or concerns if you wish.
