This may be a sign of mange mite infection or of allergies.

Dogs can develop allergies to foods, and to inhaled items, and contact allergens such as rug cleaners or chemicals including lawn chemicals or even flea bites. If your dog is not on a flea prevention that might help.
You might want to try a different dog food that has no ingredients the same as what you feed now. Diets of Fish and Potato, venison, or rabbit etc. where the protein source is new and there is no grains in the food can work for many dogs for example.

You didn't mention how old the dog is but a mature dog may also have a thyroid problem causing coat loss and skin infection.

While you can try a Chlorhexiderm shampoo to see if that clears up the sore spots I really think an in person vet exam of the skin issue is your best option at finding out just what it is and how best to treat it.
