Yes, if she is uncomfortable, possibly running a low fever (which affects her appetite and activity) has painful urination (I don't mean straining which you can see) she can feel "under the weather".
Because she was spayed it cannot be a uterine problem.

OMG! That is a huge abscess. So she wasn't licking her urogenital ares but the discharge from her abscess on the back of her left leg.
