I'm sorry to hear that Tommy's symptoms have you worried. From your description, it sounds like he may have developed hyperthyroidism, which is not uncommon in senior cats. The symptoms you describe are classic.
However, diabetes and kidney disease can also cause these symptoms, so your next best step is to have Tommy evaluated in person by his vet, have a routine blood panel done, including a thyroid test and then you will get a diagnosis and treatment, if necessary.
