can a female cat still have a menstual period after being fixed

our female cat has been fixed for 4 years ever since then she bleeds every once in awhile

If the spay did not get all the ovarian tissue a heat cycle is still possible. The solution is to remove the remaining tissue.
However if you are seeing blood in the litter box this might be due to urinary tract infection or bladder stones and crystals.
This is something you should discuss with your cat's veterinarian. If its an infection you cat may need antibiotics. If its retained ovarian tissue or bladder stones your cat may need other care including surgery.
