I have never heard of a male cat's penis being swollen - he may have a urinary blockage that is causing this and this is considered a medical emergency. You need to get him into the vet immediately as if there is a total blockage, his bladder could rupture in less than 24 hours. This situation is very concerning and I would definitely have him seen as soon as possible.
