And one of his testes is very much bigger
When he cries, its like it goes up into groin area
When he resting the bigger one is bigger than saggy

Hi, Sorry about this symptom. Please get him assessed by a surgical specialist and get ultrasound scan done to find cause of disparity in size. Some slight difference in size is common and also the position in scrotum (during play, or while crying etc) may be little different but any marked difference has to be explained. This test will also be useful for follow-up to see any progressive condition.
