i have an 18 month old who i assume is perfectly fine. no sign of fever, pain or whatever. and out of nowhere, has been screaming uncontolably for about 20 mins. It has never happened before. Not sure if we should just go to the ER

This is likely a behavioral issue. May be the start of not feeling well with a viral illness, but more likely behavioral. Try to reinforce his routine, positive reinforcement, consoling. I do not think he needs to be seen in ER.
we did move to a new house about 1 week ago. although it didn't seem to bother him. could that be part maybe of routine being out of whack?
It definitely could. Or a developmental phase "terrible twos". Try all the routines he likes to calm him. Take a deep breath. Try to be patient. I KNOW how hard that is. Put him in a safe time out for 1-2 min if needed.
