This is quite clearly a case requiring the assistance of a psychologist
Since the child is quite intentionally (likely) holding his stools
The primary reason for this is to gain attention
but the underlying cause for the same requires the help of a psychologist - preferably a pediatric psychologist
I do not believe that his explanation of not being able to wipe himself is the primary problem here
Is he otherwise constipated too ?
I don't believe that he is constipated. There is constantly bowel in his pants. I just cleaned him before school and I can almost certainly believe he is soiled again. I spend my day just waiting for a call from the school.
And that is why I agree with your assessment that this appears a psychological problem that needs counseling
Ask your pediatrician for an appropriate referral

No thanks, XXXXX XXXXX confirmed my feelings. I was trying to deal with this at home to lessen the humiliation but I undersand that it is bigger than what I can do.
It is bigger than something that you can handle alone
But the good news is that this is quite a well known medical issue
and definitely treatable
